Sassie, I have had similar symptoms as I too use a steroid inhaler. I would suggest a visit to the GP, as it is possible to pick up a fungal infection, sounds lovely doesn't it!!! I use a Betadine mouthwash, which you can get from over the counter, it is Iodine based and looks foul, but doesn't taste too bad. It will kill off any fungal infection that might be lurking, but do see your GP for a second opinion to the dentist.
The other thing you can do is fill a polystyrene (not plastic) cup with water and freeze it. If you then peel off some of the polystyrene from the rim, you can sit and massage the area without your fingers dropping off with frost bite. Check out your shoes, if they are old and past it ( hey, who isn't?) it's time for a change. If this keeps coming back, there may be a mechanical problem with your foot, find a physio and get them to take a look Rich
Neil, how old are your running shoes and do you over or under pronate? The thing with PBF is that Ultrasound/ hydrocortisone only really get to the symptoms, not the cause. If your shoes aren't up to the job the problem ain't gonna go. Have you investigated Private physio, might not be as expensive as you think Hope this helps Rich
