If you've got an acromio-clavicular joint disruption that hasn't settled after 4 months then I would suggest you see an orthopaedic surgeon with an interest in shoulders. Before you pay for physio or osteopathy go back to your GP and discuss a referral. Ask if you can use choose and book to pick out a shoulder specialist and find out what the wait will be before making your decision.

Eventually went to my physio, who identfied the problem as my top rib shifting and getting jammed into a vertabrae
